FM1/Form for court application

If after your Mediation Information and Assessment Meeting you or the mediator consider that mediation is not suitable in your particular circumstances, we can provide you with a form FM1 or equivalent. This form will be required if you wish to proceed with a legal hearing and will need to be attached your court application.
